PEBFT Accepting Appeals

If you believe you have complied with all requirements of your program, but are still not receiving your cost-sharing waiver, download a survey and appeal form and fax or mail them to the PEBTF. You must provide your full name and either your mailing address or Social Security number so that your participation can be verified. If you participated in Get Healthy's on-line program, you can log onto gethealthytools.com, click on "program status," and print your scorecard, which you can fax or mail to the PEBTF.
