Get Healthy Appeal Procedures

 

• Fax a written appeal—including your name and birth date—to 717-307-3372.  Include documentation that verifies that the health assessment was completed in April 2009 and any other documents that support your appeal.  If you did not print your health assessment results, you can do so at www.pebtf.org.  Click on “Get Healthy” then “Health A to Z Website.”  Click “log in” and enter your user id and password. (If you do not have a user id and password you will need to register.) Click “Review your Health Assessment Results.”

 

• Mail a written appeal—with the same information noted above—to the PEBTF Board of Trustees at:

• Call the PEBTF at 717-561-4750 or 800-522-7279 to appeal.  If your appeal includes documentation, you should NOT use this method; fax or mail a written appeal.
